Clinical / Ambient notes

Record the consultation. Read the draft. Put your name on it.

The consultation is transcribed and arranged under the headings a clinician already writes to. Nothing is summarised and nothing files itself, and the recording is deleted as soon as the transcript exists.

A draft consultation note arranged under Subjective, Objective, Assessment and Plan, beside the unedited transcript, with a “File this under my name” button.
The consultation arranged under the usual headings — nothing summarised, and the transcript alongside. Shown with sample data.

What it does

Ambient notes, in practice

Consent first

A recording is uploaded only after confirming the patient agreed. Without that, nothing is stored.

Two transcription providers

Amazon Transcribe for English and several other languages, with a medical model for US English — or Sarvam AI for Indian languages and Indian English.

Arranged, not summarised

Each sentence of the transcript is placed under Subjective, Objective, Assessment or Plan by its wording. Nothing is summarised and nothing is added.

Signed by a person

The clinician corrects the draft and files it under their own name. Nothing files itself.

Into the chart

With OpenEMR connected, the approved note goes into the patient’s Documents under Medical Record, with a retry if it does not arrive.

The recording is deleted

As soon as the transcript arrives — and within 48 hours regardless.

How it works

From the first step to the last

  1. 1

    Record

With the patient’s agreement, upload the consultation recording.

  2. 2

    Read

The draft comes back under the usual headings, beside the original transcript.

  3. 3

    Sign

Correct it, file it under your name, and it goes to the chart.

What it needs

Before you switch it on

Every module switches on and off by itself, per clinic, with its own screens, permissions and audit trail.

Worth knowing

It needs an Amazon Transcribe or Sarvam AI account. It does not summarise, by design: a note is a legal statement about somebody’s care, and every sentence in this one came from the consultation.

See Ambient notes working on a real system.

The demo runs every module against an unmodified OpenEMR, with invented patients. Nothing to install and no sales call.
